In 2021, through the encouragement of Neal Patterson and his family, two communities came together to create one health center -- to be a leader in rural health care. In like fashion, the two existing health foundations also came together into one... creating a stronger framework and pool of help and ideas!

The Patient Protection and Affordable Care Act (ACA) requires not-for-profit hospitals to conduct a CHNA survey of the community every three years and adopt an implementation strategy to meet the needs identified by the CHNA. The Health Foundation, along with other community organizations, partner with PHC to meet the needs identified by the survey.
